Transition and Secondary Transition

A transition plan identifies the services, supports and activities that will be provided to students in order to reasonably enable them to achieve their postsecondary goals.

Ohio Department of Education: Federal and State Requirements

Transition from high school to postsecondary options involves identifying a student’s vision of the future and the services needed during middle and high school to realize that adult goal. Members on the IEP team will identify the types of transition services needed for post secondary education and employment, independent living skills, and community participation.
