Postsecondary Transition

Federal law requires that all students with disabilities have a transition plan in place by their 16th birthday; however, Ohio law requires transition planning to begin by age 14, or earlier if agreed upon by the IEP team. The postsecondary transition plan is designed to establish goals and services aligned with the student’s Age-Appropriate Transition Assessment (AATA). Using this information, the IEP team identifies the student’s preferences, interests, needs, and strengths, and develops a plan outlining the services necessary to support postsecondary transition in the areas of education, employment, and independent living skills, when appropriate.
